Recontract of Services

  • Third party’s original NRIC/FIN

Transfer of Ownership

Mobile

  • Letter of Authorisation/Letter of Representation signed by transferor
  • Copy of transferor and transferee’s NRIC/FIN
  • Transferee: Will have to be present, as this is considered a new sign up.

Broadband, TV, Digital Voice Home, HomeHub

  • Transfer of Ownership (TRO) form
  • Letter of Authorisation
  • Copy of transferor and transferee’s NRIC/FIN

Terminate with/without a contract

  • Letter of Authorisation 
  • Copy of subscriber's NRIC/FIN 

Replacement of SIM card

  • Letter of authorisation 
  • Subscriber’s original NRIC/FIN
  • Third Party’s original NRIC/FIN

Full Mobile Number Portability

  • Letter of Authorisation

Request for Refund

Receive/Collect Handset
